how to get LMO no.?

hello good day ,
can you help me please how to get an LMO no.? i have already my HRSDC.
can we find the lmo no. in hrsdc documents?
i cant fill up my temporary work permit because i dont know my LMO no.
there are two numbers found in hrsdc doc. the
No dossier fed.
No dossier prov.
if ever,which one of this are the lmo's no?
im hoping for your reply

ang IMM 1295 better i-fill up electronically kc kailangan ivalidate since you'll need to have the barcode.
and iba naman ok lang handwritten.
basta just follow the checlklist kung anung mga forms ang kailangan isubmit.
 
Zia tnx....salamat sa pagreply ,,i already fillup the working permit at na validate na po yon, but yong problem ay di ko ma fill upan lahat,my box na nah empty kasi it leaves blank ,,pwde lagyan ko yong box after i print lagyan ko N/A sa mga na leave na blanks? the application work permit it compose written and electronically na after i put n/a in an empty blank box..it is ok lang poh? thank you thank you thank you for the reply...

my question lang ako.. bakit 2 days ang process ng Medical mo at saka pano malaman na na send na ung medical mo sa CEM?... St. Luke pala ako nag pa medical.
 
1st day laboratory exam. 2nd day physical exam. Pag wlang problema mabilis lang. Sila nagsasabi kung na forward na nla ung result sa CEM. =)
