CEC Inland AOR JUNE IMM5645 Request

Hi!

Has anyone been requested IMM5645 at the final stage of application? I received an email from CIC today. Expected "background check in progress", but got this.

I am obviously going to fill it out and send it to them, but maybe someone knows what the reason they are asking for it is?
