CEC application kit clarifications, pls help

Dear forum members,
We came to Canada in Jul 2011..and had applied for PR under 'Federal skilled Worker' program in Nov 2011 and our application got rejected since our experience letter did not cover the NOC job code description under which we applied..Now we hav 1.5years of canadian experience and eligible to apply under CEC however there are a few points where we need help from u all...they are as below
1. In IMM5406 (Additional Family Information Form) - do we need to write all names (including our family members) in Native language?
2. Do we need to attach a police verification certificate from Canada?
3. Do we need to provide my bank balance certificate?
4. Should we get all copies of documents notarized?
5. Under Work experience, it is requested to submit reference/experience letter from employees for the last 3 years. Do we need to submit even out of canada experience letter or only if we had worked in Canada?
6. I do not have a birth certificate? i dont think i can get one too since i have no contact with the place i was born. i read in this forum that a Statutory declaration will help..what is the procedure? should i contact someone for that? shud it be on a legal paper? or will a prinout on plain paper suffice?

I see nothing on the form or in the instruction guide saying anything about writing names in the native language. So, no.

Work experience in Canada only.

I will definitely read the instruction guide again.
I still see...'Complete ALL names in English and in your native language' as the 1st sentence just below the heading "Additional Family Information" Form.
Reg, birth certificate, I was born in India.

Do you have a secondary school leaving certificate (SSLC in India) which shows your date of birth and parents name correctly? Then that would be your best bet.

I am not sure if you are in Canada or India currently. If the SSLC option does not work for you, you can get an Affidavits which declares that your birth is not registered and etc. There are many law firms and lawyers are available in Canada who already know the format etc.You just need to tell them your requirement. You can also apply for a "Certificate of the Date of Birth" from Indian consulate (http://www.cgitoronto.ca/Passport/passpBirth.html) which can be submitted along with Affidavit, but keep in mind just consulate certificate alone may not be accepted.

For your 1st question, yes, you have to write all the names in the native language too!

I have my 10th Matriculation & 12th grade Marksheet with my Date of birth mentioned but it does not mention my Parents' name. will it suffice?
Iam in canada now. any idea how long will it take for the consulate to issue the Certificate of Birth? Iam concerned since the process requires submission of original passport since i am travelling out of country (to US) in the last week of Feb.

As far as I know certificate of birth doesn't replace BC . CIC accepts affidavit if you are not able to produce a document. You can prepare a letter and notarize it with a notary public.

Yes, you can prepare one as follows and get it notarized from Canada and also sent your 10th certificate showing your DOB. This is what I did

school certificate must have your parents name. Why I am telling because my friend's application returned because of no birth certificate for his wife. CIC officer wrote in the return letter that you should submit birth certificate and if no birth certificate submit school certificate which shows date of birth and parents name.

You can also see in the affidavits format parents names are mentioned. So it is must.

Forget about consulate certificate, it may take more than a month (my friend is still waiting for that since a month) and as chandn said it will not replace the BC.

Also most of the lawyers has the required letter format, so you need not worry about the format they already know this, only thing you need to pay the drafting charge as well.

Can this affidavits be notarised by Commisioner of Oaths since i dont know any lawyer here..
I can get it from Comm of oaths if allowed/accepted else i just have to google n find a lawyer to help.

are you somewhere in GTA? if you google you can find many law firms /lawyers.. Try in kijiji also. I am not sure about Commissioner of Oaths
