Hey guys, First time posting but I've been reading TONS of threads and can't seem to find what I'm looking for.
Thanks in advance for the help.

Generic Application Form IMM 0008
Q4. I am applying from within Canada, which Immigration Office do I request? Have seen some people talk about Buffalo Visa office? Is that still correct?

Q10. I have been in Canada since 2006 first studying at university, then working. What dates should I list? I have seen a lot of different answers. Should I put the "from" date as when I first moved to Canada in 2006? Should the end date be "present" as I have seen suggested? The form wants me to fill in a future date, which made me think that it wants the end date of my work permit.

Additional Family Information Form IMM 5406
Section C
I have 5 step siblings and 3 of them are married women, Should I list their maiden names or their current names? The guide says nothing here and I am not sure which would be better. Their maiden names correlate to their birth date and place but their current names correlate to their current address. Help!

Visa Office......
CPC-Ottawa
Q4. Case Processing Pilot - Ottawa (CPP-O)

Q10. Those dates are a bit weird but CIC and CBSA also have all the relevant details. Just start from the first entry. The other one is not future really, it's more so "not in the past". Basically if you put in a date in the past, that means you have already left the country, even by a single day. Just put in current or next day and then verify form.

Section C. I would suggest whatever name is on their Passports.

A: CPP-Ottawa

A: from date: first you moved in Canada; to: expiry of work permit

Hope this helps.

I have no idea what their passports say, let alone if they have passports...we are NOT close (my parents remarried after I moved to Canada) and they are still in my country of origin.
What if I tried to put their name like this: First Middle (Maiden) Married ?

Thanks for the help with the dates and processing center! Much appreciated!

U need to enter names as of present. It doesn't matter what their passport says .. u won't be asked to provide a copy of their passport at all.
So say if 3 of ur siblings r married then mention their current full name and write married next to it. But if u can then try to get their accurate address. Although it won't matter much as this is just a formality form and has nothing to do with u qualifying for Cec PR. Its just an additional info form.

The reason passport name matters is not that there's a need for copy of passport. Passport name matters because firstly, it's the correct English spelling, or at least generally agreed to be correct. Secondly, if they travel to Canada at any point in time or CBSA cross-references their names with another border authority the name on passport is what shows up. When background check happens and they look into their lists, the name on the passport is what shows up, etc. It's your international identity document.

Should be fine, you fill the forms to the best of your knowledge. Put one on the form and attach a paper and explain the other name(s).
