Card Renewal Form (IMM 5444) - Signature at bottom of form

Hi friends,

I have a query hoping someone can help me with.

At the bottom of the IMM5444 there is a section (Section 8 - Consent and Declaration).

It asks for:
1) Signature and date
2) City (Required)
3) Province
4) Country (Required)

My question is: Am I required to input my current address underneath my signature again?
