What does “An original recent ID photo, passport format (35 mm x 45 mm), with your name on the back and
stapled in the box opposite.” Mean I am confused how do I staple the photo is the back side (blank)should be upwards on the form or just how we generally paste or staple the photos on the forms!

Just write your name on the back of your photo and staple the photo from the top onto the required paper. It should show your photo on the front. They will probably rip it off to see your name or just lift it from the bottom.
