cant save medical request form...please help

Guys i am applying online but i cant save medical request form "IMM 5753E" any one know how to do it???

Actually it appears on my application portal where i have to submit documents , in this form there are only two options whether i have to undergo medical exam Yes or No, i tick yes becoz its mandatory for pakistanis but i cant save that form so i can upload it again on required tab.

any one who faced this issue,??

You can scan the document after ticking the option and upload it in pdf format. I have the document too it does not saves changes so there is no other option.

you can pretend to print it and save as pdf
