Cancel Representative Form Confusion!

I have a representative but there has been countless issues with him and I'm starting to get really fed up with his unprofessionalism.

I know I have to fill out a use of representative form and cancel him but I (sponsor) want to be the representative for my husbands application. Do I need to fill out only one form and cancel the representative and add my info on the same form or do I need to fill out two separate forms?
