now what a stupid thing i had 2 passports from different countries non of them has this stupid thing as guarantors , or references , this is some north korea stuff going on on canada
Don't shoot me, I'm only the messenger. This seems to be the norm in the Commonwealth countries like UK and Australia as well.

Did you have to apply in person for those 2 countries' passports?

Not sure if this is the correct thread, but wanted to check a couple of questions in passport application
1. Part 5- Identity - It says write name as appears in document . I am using Ontario Driver's license . The name format is last name, given name.
Should I use this format or given name-last name
2. Part 7 - Address
Address in the last 2 years - I have moved to my current address this year.
Do I need to write my current address again ? for "to date" should i just leave blank ?

1. When writing your name, either writer it as Last Name, First Name (with a comma) or First Name Last Name (without a comma).
2. Write: "Current" for To date.

If you are submitting by mail, I suppose the guarantor must sign the copy of the ID as well. It says so in the instructions provided in the application form.

Hi

In the passport application, there is a field for " mother's Surname at birth", not sure why its there.

does the Canadian passport biographical page also indicate the mother's last name on it ?
