Canadian Citizenship application for Minor

Hello,

I have a question regarding Citizenship application for Minor applicant, both parents (me & my wife) and my son are Permanent residents.

Which form CIT 003 or CIT 0403, do I need to use when I apply for Citizenship for my son?

And for Photocopy of '2' pieces of your child's personal identification, can i use one as his US Passport and for another can I use his SIN or the Province issued health card?

Health card and school index card

Passport data page is now listed ...they updated the instructions june 30, 2020. So you can use passport plus health card no need for school index if you don’t have it

You already have health card and passport hence they are sufficient now.

  • school record;
  • health insurance card;
  • a passport page containing their photo and personal details;
  • a hospital or immunization record, may be provided.
 
Hi everyone, please I need your help around police certificate for my kid. He spent 250 days outside Canada after landing as PR. For most of the 250 days away, he was under 14yrs. After he turned 14, he spent only 95days before returning to Canada fully. Would he need to provide a police certificate? Thanks for all your feedbacks!
