Canada visa rejected reason being my previous entry to Canada was not declared.

My previous entry to Canada happened accidentally during my USA stay.

During US stay visited Niagara fall unknowingly entered wrong way which led to Canada border.

Could you please suggest an optimal phrase with this information. So that i can express same in my next application.

When you crossed Canadian border by mistake , Did border petrol officer gave you anything in writing or did they noted down your detail ?
When did this happened ?
