As the main applicant, I have my passport and my son has got a passport as well. The delay is since my wife applied for her new passport in January we have not received it yet and the date the gave me is 5/4/12 and there is no way that i can meet this dateline.

So email VO and explain to them, they would understand. You are thru the process...... all the best!

Something just occured to me: has VO already sent you PPR? If they haven't already, I will suggest you don't tell them anything. Just concentrate on getting a new passport for your wife. If they send PPR before she gets her passport, then you can send th extension request. This is to avoid unnecessary delay when you tell VO not to send PPR just yet.
