On august 7 I got a passport request and was very happy. The next day I received another letter saying my visa was denied because I failed to present a medical examination which is a requirement... that is obviously a mistake because I got examined two months ago by one of the CIC doctors, I got the medical examination request 1 day after my online study permit application (june 15, applied on june 13)...

I never received a medical examination reception confirmation from CIC. Do they send one? I did, however, receive a mail saying they hadn't received my examination yet (about two or three weeks ago), but I thought it was a mistake. Apparently it wasn't...

Do I mail CIC? Do I call the doctor? what?

Do I need to apply again? I will need to pay 500 USD if that's the case! Which is something I would not want to do since it wasn't my mistake in any way.
 
If you've already done medical exam, email them that you already have done it. Be sure to attach your official document of medical exam or any proof of evidence.
However If you still get rejected, don't worry.
I heard someone who got rejected caz of same problem as you have, he solved by reapplying with old medical exam and explanation with it.
 
But does that mean I have to pay 125 cad again because of something I didint do (or in this case, something i DID)? that sucks!
 
You should have replied to the mail you got two weeks earlier your rejection. They sometimes don't get the medical but if you had replied with the receipt then you wouldn't face rejection first place. You can try emailing CHC about this matter. But I think you have to reapply and give the application fee 125$ again.
