We had ours extended automatically back in May for a year. They never sent notification. Only way we knew was that we ordered GCMS notes. My partner's medicals expired at the end of July but we submitted it upfront since we are January applicants. I believe, now they request it to be done and they won't accept it upfront anymore.

That's awesome. Do you where to find official Operational Bulletin for medical extension? The old one named 523 and dated on May 24, 2012 is already expired tho.
 
I would say that medical extension is automatically given until CIC requests it otherwise, whether the annual medical test is currently valid or not. The 12 month extension is not the rule. It is up to the agent to determine the length of extension.

I'm not sure what the Operational Bulletin for medical extension is and what does the number "523" refer to? We never asked for an extension. My partner has no health issues, but I see from others' posts that that doesn't always guarantee automatic extensions.

When I googled Operational Bulletin 523, it referred to the TFW program. Does that apply to the Family Sponsorship program? As far as I know, there isn't a set list of criteria whereby officers can systematically decide whether an applicant will have the validity of his/her medical results extended. It is at the discretion of the Visa Officer.
