Yes, include one for your spouse too. My husband's nearly was rejected because they processed his before mine. They actually rejected it then reopened it once they read my letter mentioning his application. It was a very scary 15 minutes for us.

I am writing in regard of my application for a BRIDGING OPEN WORK PERMIT (Name: XXXXX, Date of birth XXXXX, UCI XXXXX, Application Number XXXXX, AOR attached).

It is my understanding that I am eligible for a BRIDGING OPEN WORK PERMIT accordingly to OB485, as I have applied for Permanent Residency (CEC Program) on date XX.YY.ZZZZ, have received the Acknowledgement of Application Receipt (AOR, copy attached) and my current work permit is expiring within 4 months (attach copy of current work permit).

For your spouse you can write the same letter but refer to your BOWP application and your PR application. Obviously mention that spouse is therefore eligible too.

OB485 has expired. I referred to the "International Mobility Program" and mentioned the URL (http://www.cic.gc.ca/english/resources/tools/temp/work/prov/bridging.asp) that states the eligibility parameters.
