The worst thing a visa-exempt applicant can do while in Canada, is submit an inland app. Times are getting worse and worse, now at 16 months for stage 1 + 8 months for stage 2 = 2 years to get your PR.
A straightforward app through London visa office can be done in 8-10 months total. So even if you're residing in Canada at the time, still definitely submit an OUTLAND application.

Assuming you make the right choice and apply outland, you are supposed to do the medical UPFRONT so submit it along with the application.f pregnant, is it possible to postpone the medical until a safer stage of the pregnancy? About how far into the application process do they request the medical?
If you're pregnant and don't want to do x-ray, the medical portion of your PR application will basically go on-hold and sit there until you finish it.
In general, I can see if you don't want to to submit the app while traveling as it can be hard to gather all the required documents. My best advise if you want to wait until you get back to Canada, is to
1. Once back in Canada in March, submit your OUTLAND application asap
2. Submit it before you get pregnant so you can do the full medical exam upfront
3. Don't waste time waiting for updated docs in your married name, just submit everything in maiden name
