Bridging OWP-online application-Schedule 1

Hello everybody!
I am applying online for a bridging open work permit, and there is a "schedule 1 application for a temporary resident visa made outside Canada" document that needs to be attached. Just to make sure, is it IMM 5257 Schedule 1 from here www[dot]cic.gc.ca/english/information/applications/visa.asp ?
And also did you attach this doc? This questions are mainly for people who already applied.
Please advise.

Yes i attached..it is a simple form, so attach it, don't take chances. (It is mentioned as optional though)
