BOWP - form 5710e

Hello Guys,
My BOWP just got rejected and I'm not sure if I made some mistake in my application. I checked "apply for a work permit with the same employer" in question 3 In form 5710e because I couldn't check "restore my status as a worker" and I left all information about my employer blank to get open work permit.
Which checkbox should I have checked?
Their answer was that my permit can't be approved by section R203 because I didn't possessed a valid LMIA.

Have you already received ITA under Express Entry and submitted your complete application? If not, do you have a provincial nomination?

Did you select by any chance in details of intended work section 6th question is about do you have LMIA or offer letter?did u select yes or no?
