I have used the below format as a declaration by my father from my home country. It worked out for me, you can either get an affidavit from your home country or with in Canada itself which should work. All the best.

1. I submit that I am the deponent herein and I am a citizen of XXX presently
residing at the above said address.
2. That I have direct personal knowledge of the event and circumstances
concerning the birth of XXX as I am his father and his particulars are as per below.

Date of Birth : XXX.
Place of Birth : XXX.
Father’s Name : XXXX.
Mother’s Name : XXXX.
3. That I have known the applicant (XXXX) since XXXX.
4. That this affidavit of firth facts is offered as there is no official record of birth
and I submit this affidavit for the purpose of attesting to the birth facts of
XXXX.
5. The contents of this affidavit are true and correct to the best of my knowledge
and believe nothing is concealed.
The above contents are read out to me and I found them correct. Also copies of
educational documents, passport and permanent account number of the applicant
are verified and confirm the above said facts.
