Sorry i forgot commenting on that part -- YES please affix that sticker on to your package as well as to your application - thats what i did - it helps them in putting you into the appropriate category.

Well, Initially I have fixed that on to the package. Now I don't have one as it is already consumed for first time so I was wondering how you did that in your case when you re sent the application, Thanks in advance.

Sorry to bother you, but have they given more than one for you? In my case they have given me only one.

I don't think it really matters and I should not be worrying for it. Also shall I send the explanation letter sent by CIC in my application. Thanks!

Hi,

I have used the below format as a declaration by my father from my home country. It worked out for me, you can either get an affidavit from your home country or with in Canada itself which should work. All the best.


AFFIDAVIT

I, XXX aged XX years son of late XXXX, resident of house number XXXX, do hereby solemnly affirm and state on oath as follows:

1. I submit that I am the deponent herein and I am a citizen of XXX presently
residing at the above said address.

2. That I have direct personal knowledge of the event and circumstances
concerning the birth of XXX as I am his father and his particulars are as per below.

Name : XXXX.

Date of Birth : XXX.

Place of Birth : XXX.

Father’s Name : XXXX.

Mother’s Name : XXXX.

3. That I have known the applicant (XXXX) since XXXX.

4. That this affidavit of firth facts is offered as there is no official record of birth

and I submit this affidavit for the purpose of attesting to the birth facts of

XXXX.

5. The contents of this affidavit are true and correct to the best of my knowledge

and believe nothing is concealed.

The above contents are read out to me and I found them correct. Also copies of

educational documents, passport and permanent account number of the applicant

are verified and confirm the above said facts.

Witnesses: Signature of deponent.

Solemnly affirmed and signed before me.

Hi Sai Sivaram,

I have given my medicals, it may take 10-15 days for doctor to submit them to CIC. They have requested me some additional information like police clearance certificates and RPRF fees. I have gathered all these information to submit.

They have mentioned in the email that I should be sending these to CIC Sydney, NS office again( I have submitted my initial application to Sydney itself). I have heard from my colleagues earlier that once they accepts the application the Visa processing office will be changed to Toronto and our additional documents needs to be sent to new office. I am confused whether to send to NS office or do I need to check with them regarding Toronto office. Do you have any suggestion and what is your experience in this regard.

I have gathered police clearance certificates for 2nd time now, I have submitted them along with my initial application. They have requested them again as additional information. So I want to make sure I am sending them to the right place.

I have called the call center number and sent an email clarifying on this but there was no response to my email and call center guys said they wont be able to help as they consider my application as outside of Canada. Though I am with in Canada and applying under provincial nominee program from Manitoba under skilled worker. So I am not sure how to get clarification from them even now regarding the correct office where I should submit my documents.

It is time consuming activity to get police clearance certificates for my country and that too they wont give more than a copy at a time, hence the cause of dilemma.
