Birth Certificate Vs Passport - Name and Place of Birth do not match

Hi All, Could anyone please help with the below query? The Name and Place of birth in my Birth Certificate are different when compared to my Indian Passport.

DETAILS IN MY BIRTH CERTIFICATE:
Name: R. PRAVEEN KUMAR
Place of Birth: SHEELA HOSPITAL
Name of Father: S.C.RAJA SEKARAN
Name of Mother: DURGA DEVI
Birth Certificate is issued by COIMBATORE CITY MUNICIPAL CORPORATION, COIMBATORE.

DETAILS IN MY PASSPORT:
Surname: SAJJA RAJASEKARAN
Given Name: PRAVEEN KUMAR
Place of Birth: GANDHIPURAM TAMILNADU
Name of Father or Legal Guardian: SAJJA CHANDRAIYAN RAJASEKARAN
Name of Mother: DURGA DEVI

The Place of Birth SHEELA HOSPITAL is located in GANDHIPURAM, COIMBATORE, TAMILNADU.
In my 10th, 12th Std Marksheet & College TC, my name is "PRAVEEN KUMAR S R". My Date of Birth is the same everywhere.
What are the options and the documents should I submit along with my PR application under CEC so my application gets through without any hassles?

I could not post the whole content in one go so I have posted in 3 posts. Please help!

This looks fine, there will be no problem...go ahead! Its minor...
Place of Birth is determined by BC, Passport just proves your nationality...
If you want, get an affidavit attached explaining everything...
