I am from Chennai, India.
It seems I can show Affidavit or Statutory declaration for non availability of my birth certificate.
May I know the format of Affidavit or Statutory declaration that is accepted by CIC for FSW 2014 PR program?
May I know from whom I can get the Affidavit or Statutory declaration for my birth certificate?

Is there any other option available which can be accepted by CIC apart from Affidavit or Statutory declaration?
 
Get affidate from notary public from your area. Also SSLC certificate with DOB and parents name also acceptable.

Thanks a lot. I have SSLC(10th standard) mark sheet only and did not have SSLC(10th standard) Transfer certificate.
Also SSLC(10th standard) mark sheet contains only my name and date of birth without my parents name.
Is it fine to submit SSLC(10th standard) mark sheet alone?

If SSLC(10th standard) mark sheet is not acceptable as it does not contains my parents name, is it enough to submit affidavit alone from my area's notary public as you said?
 
Parents name is must. Yes make affidavite thro notary public.
 
Is there any CIC prescribed affidavit format to get the date of birth proof?
If so could you please share the same?
