Birth Certificate + Passport info mismatch

Hi Guys,

I was born in India(Indian birth certificate) and moved to UK at early age, now that i've decided to apply for PR I've checked my birth certificate and it doesn't contain my family only my name and fathers name. My passport has my name, my fathers name(middle name) and my family name.

1.What are my options as the surname is missing?

2.My native language is Gujarati, however I can't read or write it. Therefore on the application form where you have to enter details in english and native language, can I just use english?

As your passport is with your full name, you can submit a solemn affidavit attested by the Notary public for the omission of family name on birth certificate.

I'll also have to get my certificate translated and a affidavit from translator, can I get another affidavit from the translator that the surname is not on the certificate or does it have to be from someone else?
