Dear Friends,
My daughter does not have her name in her birth certificate. It just mentions that a baby was born to parents(father's and mother's name) on xyz date at abc hospital.
Can any one please share the content or format of affidavit that I need to submit stating that she is my daughter.
Secondly I don't have personal certificate for myself. I have class Xth marksheet as proof of DOB. I have passport, PAN card etc as supporting documents to prove the same. Can any one share the content of affidavit in my case for not having birth certificate?
