Birth Certi issue with two name

Hello,

I want to apply to sponsor my spouse. She have two name in her birth certificate and all other documents only have 1 name.

She had done affidavit during the time when she added name into birth certificate so is it ok to include English version of that birth certi with two name or CIC will ask for affidavit and other documents.
