You don't need any forms for medical now a days.

Just take your passport, two copies of it, 5 pp size photograph and email letter from NDVO, to your DMP

Passport
RequirementsWe require an original VALID passport for yourself and every accompanying family member.

Passports for all accompanying family members must be submitted together.
Passports must be valid for at least the next 6 months, or at least until the date of your intended travel to Canada.
There must be at least one blank page in your passport. If there are children included in your passport, there must be blank pages for their visas as well.
The immigrant visa that is to be placed in your passport will show an expiry date. You may enter Canada anytime before the expiry date shown on the visa; this date cannot be extended. If you fail to enter Canada before this date, you will be required to reapply and submit a new application and processing fee.
Please note: Immigrant visas cannot be placed in Diplomatic, Official or Military passports.

Right of
Permanent
Residence FeeEvery principal applicant and his/her accompanying spouse/common law partner is required to pay a Right of Permanent Resident Fee (RPRF) in the amount of $490.00 CAD (or $490.00 USD).

The RPRF must be submitted to this office in the form of a money order, certified cheque or bank draft payable to the “Consulate General of Canada” in USD OR to the “Receiver General of Canada” in CAD. NOTE: Fee paid at a financial institution in Canada or fee paid online is not acceptable.
The RPRF will be refunded if for any reason you do not immigrate to Canada.

Settlement FundsBe advised that when you enter Canada as a permanent resident, you will be asked to provide proof that you have sufficient funds available for settlement. Photograph Specifications

The photograph must:show a full front view of the person's head and shoulders showing full face centered in the middle of the photograph; neither smiling nor frowning;
have a plain white background;
be identical (black and white or color) produced from the same untouched negative, or exposed simultaneously by a split-image or multi-lens camera;

**Photographs must not have staple holes, stamps, creases or tears in them.

The frame finished size must be 35 mm X 45 mm (1 3/8" X 1 3/4").

The photos must show the full front view of the head, with the face in the middle of the photo, and include the top of the shoulders.

The size of the head, from chin to crown, must be between 31 mm (1 1/4") and 36 mm (1 7/16").

Crown means the top of the head, or (if obscured by hair or a head covering), where the top of the head or skull would be if it could be seen.

Send all the application forms (remember some forms should be filled out for each accompanying member).

2 address labels, one with your address in your resident country's official language and the other in English.

A check/cheque for the application fees.

If there is not enough space on the application form, you can write it on an attachment. Be sure to say your name on that attachment, the name of the form and the question number.

If necessary, write a cover letter to say what you have included.

You don't attach your IELTS to your initial application to Sydney, NS. On one place it says to attach it and on the instruction, it tells you to send nothing else. So I believe you don't have to send it now.

However, if you are claiming language points based on where you come from, your education, etc, then you might want to write a letter saying why you are claiming your language points and send that to Sydney, NS with your initial application. If they don't need it, they can throw it away.

Some tips for the prospective applicants appearing for interview.
- Carry along with you the xerox copies of the initial application filled by you.
- Carry original of all your educational certificates and job offer/contract letters mentioned in the application.
- English language test results copy.
- Passport
- You may be asked questions about your field of study.
- Your present job responsibilties and other details to ascertain you can settle down in canada under your job category.
- Before the interview date do some research about the city where there is maximum demand for your trade.
-Be cool and don't bluff. Speak the truth

hii, i planning to submit the doc in Jan 2013, i'll submit my wife's doc. she is a bachelor in science, microbiologist, she wes working in Egypt for more than 6 years in hospital lab,
>>> so i need your help to provide me with a " work Experience letter format" the accepted format by CIC to ask her manager to prepare it as soon as possible.
she is not workning for the last 2 years cuz delivery and got baby.
she has experience in egypt from 2002 to 2010, near 8 years..
