Hello,
I am working on obtaining dual citizenship since my father was born in Canada. He was born in the 50's and the only thing available at time were baptismal certificates. In order for me to start process so I can get my proof of citizenship along with his is to get his baptismal converted to birth certificate. I have the application for Civil Registrar filled out but have 2 questions.
1) What is the difference between actual given name, usual given name, and other given names. When I put this on app do I put the first and middle given names first followed by surname even though I already put surname on left? What is the difference between the given names. I.E. John Henry Pierre Hancock so would it be
- Actual: John Henry Pierre Hancock
- Usual: John Henry Hancock
- Other: Henry, Pierre
I want to make sure I am filling out form correctly. On all of his US identification it shows John Henry Hancock, no Pierre.
2) Is his birth already registered on file with civil registrar in Canada ? Since he only has baptismal certificate is this something my grandma would have had to have done on her own? He attended majority of his schooling up until 16 while in Canada and came to US after that. I just want to make sure we should be requesting a long form certificate of his birth and not going through the process of declaring a birth now almost 60 years later.
Please advise, thanks
