I tried to contact caips@indigoconsults.com for details about ordering CAIPS NOTE. They have replied as follows:

Thanks for your email. To order CAIPS report, you will be required to
provide information to complete your CAIPS form. Once your form is ready
then we will send it to you for your signature.

We require your complete name as on your application form, Immigration
File No, Address mentioned on the application and phone number. It
normally take 3-5 weeks for the CAIPS report to come through email. Once
we receive it, we will notify you.

Our charge is $20. You have to deposit $5 (Taka 400) before we process
your CAIPS request and the rest (Taka 1250) you can pay when we receive
your CAIPS report. This amount can be deposited in Brac Bank. I will let
you know the bank acct# once you send us the information requested by us
for completing your form.
