Hi all,
I have a question regarding Background declaration form. I have a valid visitor visa to US and I have made a lot of road trips to US while in Canada. Should I declare the road trips to US in this form? Honestly, I dont remember some dates of entry/exit.
 
Not on background dec.

On Travel form.

You need to give as much info as you can, referring to stamps etc where possibly. Check visa statements etc to help.

Is your application still in process ? I guess you applied in 2011.

My application is in progress still. However, I am being asked to fill Background declaration form (link below) again now, I dont know why.

This is the form I am asked to fill.

We have to fill all travels done in section 9 of this form. My question is whether to cover US road trips in this or not.

As I've said - yes.
 
Passport should have all the entry/departure dates. You just have to mention them in the form...

No...many ppl dont get stamps/dates or anything else on the passport when entering/exiting US...
