Background check questions in PR application general form

My US B1/B2 visa application got refused last year . Should i mention in the Background application form question e = "Are you refused or denied entry to any country"
Should i also submit the application refusal form obtained from us embasssy in my PR application ?

I may be terribly wrong but doesn't that statement mean getting denied at the Point of Entry rather than a Visa application?
