I am very confused about where do I put my name and dob on the pictures. Do I put them on the back of where the studio stamped? I wrote my name on the guarantors signature line and put my dob underneath it. Not sure if this will be acceptable. There is no clear instructions on it.

As long as your full name and date of birth is stated clearly somewhere on the back of one photograph it should be fine. Make sure there is also the date sometimes the photographer doesn't put that.
