Bachelors of Engineering from India, Help needed regarding WES-URGENT

Hi ,
Today I going to send docs to WES.
I have transcripts with me and photocopy of my Provisional Degree Certificate.

In the required docs check list, I found the "doctoral programs-Confirmation of Award/Degree Conferral" , for that what doc should I send and how?

I do not have the Academic Records Request Form .
Please help!

What education you did?

If it is bachelor, then you required to send bachelor's document only.
If it is bachelor plus masters, then you required to send bachelor plus masters documents.
And if you did doctoral which is PhD, then you need to send above two plus PhD documents.
