B4 Form " goods to follow"

Hello Friends ,

I am planning to do my landing this August and in the process of getting settled out and filling the Form B4 (Goods to follow). Can some one help me out on how to fill the form. Please respond to my questions

I have some gold jewelry to be taken , Can I self appraise or should it need to be appraised from an authorized jeweler ?
Do I need to take bills and photographs ?

Finally , Can someone share a photocopy of their b4 form as a sample , could be helpful for me to fill mine
