my in canada sponsership has been applied in june 2018. on oct 2018 i got an exclusion order because i worked 2 days on cash. then i came back to usa. now my question is this that i received an open work permit. So do i need to fill a separate form of ARC or just simply send the fee to them?

You need to submit a full ARC application. Just sending the fees isn't enough.
