This is the August spreadsheet from 2012. You need to find the new one for 2013 because most people from 2012 have received their visas or will be shortly. \

Hi Guys.

Can someone please advise me on the following:

My wife received the Passport request by mail. There was also an Appendix A that she needs to fill out and send along with her Passport. In the Appendix A, there are columns to be filled out for Applicant, Spouse, Children etc. Since I am the sponsor & her spouse & live in Canada already and there are no children, she is the only one that needs to fill her information under the Applicant column in this Appendix A, right? (I guess if this was under FSW category, then the spouse information would be there since both of them would get visa at the same time).

It is most likely just her, since she's the applicant and you don't have any children coming along with her.

However, if the Appendix A comes from the country specific application papers, then we can't be sure, as each country specific forms will be different.

No it was not a country specific form. It was to be sent along with the Passport.

I had no idea about this I just printed mine normal with an ink jet printer, but they seemed good enough, not blurry or anything like that. I hope they're okay.
