Can you please advise what form number is the consent form?

IMM5744E is that form. A Canadian citizen/ PR holder living in Canada, is required to attach it while making on-line request of gcms notes on behalf of other than him/herself.

So, in order to get the notes for stage 2, I basically need to attach this consent form as a scanned image, right?

Still I am confused. Because, am I ordering the gcms notes for myself or on my wife's (applicant) behalf?

Your wife (PR applicant) has to fill up and sign this form, so that you can request gcms notes on her behalf. You have to prove your presence in Canada, for that I attached scan copy of my Canadian passport and that worked, but some guys advise to attach driving license too. Remember, you have to pay C$ 5/ as processing fee.

I was planning to do it again, but today we received email from CHC New Delhi, to pay RPRF (landing fee) and to mail payment receipt and passport together to their mailing address; and hence we will delay it now.

Thats a TEMP tracking #, they will email you another # but its not track-able - in other words, you can give tracking # with ATIP for communication or questions
( I don't see any other tracking site )

i got a temp tracking number now. How long do you take to get another#?
