ATIP request, help required :(

I have requested someone resident in Canada to file ATIP request for me and my wife (we both applied for study visa in Jan 2017 that got rejected) . Anyone please answer below mentioned questions:

1) do we have to attach one IMM5744E form or two? (as there is an option in the form for spouse/ husband).
2) What other documents are required to attach along other that (consent form, Passport, and immigrant card)

I shall be thankful for your help.

I'm not sure about the first question whether you need to attach separate forms or a single one, but the second question is simple, your requester must provide a proof that he/she is presently in Canada so the single document, like PR card or Passport, will be sufficient.
