Hi, I am the sponsor.

Do I need to provide CIC with my own Medical exam report? and a Copy of my Marriage Certificate?

And I know for sure my wife will have to get medical exam, but does she have to enclose a copy of marriage certificate in her application package?

As the sponsor no you don't require a medical . You only need to send 1 marriage certificate in your package . If it is not in English or French you will need it translated and notarized.

I believe a copy is accepted. Most "original" marriage certificates are really copies anyway.
 
The sponsorship guide and the country-specific guide for your wife tell you everything you need to include. If the guide says to send copies, you send copies. if it says to send originals then that is what you must send. Every country is different. Follow the checklists and you can't go wrong.

All supporting documentation (such as the marriage certificate) that is not in either English or French must be accompanied by a certified translation. So, for your marriage certificate they want to see the original (or copy of the original) along with a certified translation.

If a document is translated and you are not including the original document, then you must include a certified true copy of that document, not just a regular photocopy. This is true for any document that needs to be translated, no matter what it says in the country specific guide.

Translation of documents

- an affidavit from the person who completed the translation; and

Most police certificates themselves are valid for 1 year, I believe. BUT CIC will only accept the police clearance if it is submiited to them within the first 3 months after the police certificate issuance date, regardless of validity life of the certificate.
