Applying for Spousal work permit while Inland PR in process

Hello,

I am applying for my wife Spousal work permit. She is currently living with me in Canada as she has a US Green card and I have also applied for her PR -Inland. When I go to work permit application and go through all the steps, the form it gives me is IMM 5710 Application to change conditions, extend my stay or remain in Canada as a worker. Is this the correct form for Work permit? Kindly help thanks

Yes medical and biometrics are also done. For some reason they moved the application to etibiko office and now its taking for ever thats why we are applying for work permit
