Remember to fill out the PRTV form, family info form, and a letter explaining your reasons for your application, and bring along TWO passport-sized photo and your original passports.

What is the family info form? Have not seen it in the application package. Its just me that needs a PRTD, my wife and son are both Canadian citizens and are back in Canada. I need a PRTD because i did a soft landing and went back before i got my PR card.
 
Hi Guys,

My wife landed in Canada and due to emergency reason she traveled back to India in a couple of weeks. She is planning to come back again after a month or so and doesn't have a PR card. I am planning to apply for PRTD , but confused with the steps 8 , 9 & 10 mentioned in the checklist IMM5627. Does steps 8, 9 & 10 are applicable for the person who just stayed in the country of 2 weeks?

Dont worry about that, just show them what you have like the COPR and write a letter explaining her situation.

Did you apply to PRTD from Saudi Arabia ? I'm currently in Saudi Arabia and i need to apply to PRTD for my wife, please could you help me as i'm confused in filling the form and have 2 questions on it, if you can answer my queries i'll be appreciate

Hi mIrfan81 how long did the PRTD process take from submission to getting the passport in hand? My spouse is in similar situation and processing time information is lacking
