applied for extension vistors visa, got two letters back, one with the actual visa extended till July, 11, and on the bottom of the visa says medicals have to be done within 90 days. The 2nd letter is the medical form. My question is am I approved or does it simply mean pending till medicals is passed/failed? What is the point of doing medicals with a 5 month extension? Cash grab?

1. You are approved, but if you don't have the medical CIC will rescind it.

so the only way for them to rescind it is I have major illness or they do not received a medical report from a doctor within 90 days of the letter? Is the 90 days from the date on the letter or from the last visa expiry date? My orginal visa was till Feb 11, and I wanted to stay till May or June, if I left in April(within the 90 day period) without doing a medical report technically I'm still legal and won't cause any problems next time I apply for another vistors visa? Because I dont see the point of spending $1xx.xx for a checkup just to stay a extra month.

Yes, except that the next time you apply/enter as a visitor, CIC will see that you didn't bother to have the medical, and decide that you won't obey CIC rules and make their decision accordingly.
