No it is not 2nd AOR. Status changes to "Received by Visa Office" when visa office get the paper files from CIO. 2nd AORs are not issued until the visa office starts processing the background checks for your file.

Usually medical requests are sent within a couple of weeks of 2nd AOR if your case is straight forward. However, currently only a handful of visa offices have started sending 2nd AORs.

As for email or snail mail - it depends on your visa office, since they vary.

Most of us have received it through snail mail though it is the sole descrition of your VO how they wanna send it.
