It's better to fly with Air Canada I don't know about other city but they fly from Karachi Airport.

Last time in Oct I visited and they offer me 48,000rs for Feb flight which was really cheap and was Air Canada
but the maximum they told me will hate too 80,000rs in end days but will drop in the month of Dec as for Christmas...
I ask for Toronto may be they have connecting flight as well to other city as well . .

I visited United Travels . . . .
They were giving in Oct . . . Don't know about today
It's better you call them and ask them will give you clear picture . . . .
But right now focus on your medical call not on Air ticket . .
