I had a leg surgery due to an accident in 2004 .... should I take the medical reports with me of that case???

Yup. This is the number. You will have to take your passport, 5 recent passport sized photographs and the medical exam letter. No other document is necessary.

About your leg surgery, mate I will suggest you to take your medical reports with you from that time because they usually ask that if you have undergone any surgery or operation that required stitching or some other long term ointment.

You do not have to worry about medical examination mate, it is usually a smooth process and does not require any special preparation. Good Luck!

i have applied on 19 november.just received call from tcs to collect passposrt.i will collect it on monday n tensed what it is...medical call or rejection
