I have looked for a lot of opinions and answers; does form IMM5707E (family information) needed to be signed? Fill, print and sign the application in order I can upload it? or just leave the spaces in blank?

Print the document with all of your information, sign it, scan the document and upload it. I personally did that, and I was being advised by a ICCRC Consultant.
