application form question

Dear All,

I have several questions regarding the application form for simplified process (imm0008e_SW):
1. Item NO.11 - as per rules, the blank should be filled in with N/A, or the application would be returned. shall I leave the birthday blank?
2. Item No.17 - Your Identity Card number. I have two identity card number, one is Chinese ID, the other one is from the country where I am currently residing in. Which one I should use?

The answer will be really appreciated.

If your passport is chinese, you should use the chinese ID, to be coherent.

(but that's just my suggestion, wait for the opinion of someone more knowledgeable)

I put N/A on the birthday of my dad and mom because they just have year of birth. I think that is exactly correct, right?
