Which is the box they are referring to ? Where do we have to write NONE or N/A or "I have never been a member of an organization or association".

Could some one help please ?

If you have never been a member of an organization or association, write the exact sentence from the instructions where you would normally write the name of the organizations.

1) What about the unfilled i rows where you mention your activity, old addresses, names of children etc. ?
For instance while filling "List all addreeses" or "work expereince" we filled in the first 2 lines and there are many blank lines left. How to handle them ? Should we mention 'NA" in those lines.

2) Same quesion on sur name. I do not have surname (even in my passport). so while filling surname should I mention "None ?

I'd be very grateful if you can answer.
