I am in the film industry and as such each project has a separate LLC Production company formed only for the run of principle photography "employer" but only two major payroll companies.
Question 6. F asks to list work and education history for the last 4 years. There is not enough space to include every production company AND the fill in temporary agency work for the last 4 years. Should I attach a seperate sheet with all the productions and thier information to the form? If not how should I go about answering this question?
