Yes, I did write a full cover letter (with file no. and UCI) specifying the attachments, translations, etc.

Last night, eCAS changed to "Decision Made"

On the outside, it changed from "In Process" to "Decision Made",
on the inside it now looks like this:

Permanent Residence
1. We received your application for permanent residence on <date>.
2. We started processing your application on <date>.
3. Medical results have been received.
4. A decision has been made on your application. The office will contact you concerning this decision.

The Visa will be stamped 1 year since your MR done. At least several months need to prepare for landing. Y will get the resident fees and passport promtly, unless they found some trouble in your documents. I am of opinion that you have waited since 2011, since couple months do not make sense.
