They don't call for medical if they decide to refuse the application. So don't just confuse yourselves with this misconception of refusals after medical for no specific medical reasons.
There were two cases last year, who got rejection after medical.
1) A student got rejection after his medical, reason was travelling history. He traveled to Canada in the past.
2) A student got rejection after his medical, you didn't respond our email.
Actually, I you have made passport just before your submission of study permit the VO is doubtful about your travel history or having another passport. The visa officer asks through an email regarding the said issue and the applicant has to respond back only with an email. But, the second student responded through email but his email didn't reach CHC due to technical problem.
The same happened with my cousin last year, he made his first passport just before submitting the permit. But he replied using all three mode of communication i.e fax, email and post mail.
So there is possibility of everything rather than only a medical defect for rejection after medical. I hope this helps.
