Appendix B form needed for PPR

Need help guys. I called the CVAC/VFS office here in Philippines and asled for help re: service fee, etc. The thing that made me confused was when the customer service agent told me that the Appendix B form sent thru my email by the embassy of canada consist of 6 pages form when in fact its just a 1 form. Its not in PDF form or an attachment so i can download it but its incorporated in the email.
Ill try to attached a link of the screenshot of our appendix B form. If its OK can u guys show me how your Apendix B form Looks like?

Hi. Cant view the attachment. Ang itsura nyan ay pa table. Na may nakalagay na Appendix B. Ang info sa row ay applicant, spouse/partner, child 1-5. Ang column naman ay family name, given name, dob, pob, ms, gender, passpirt #, date of exp, ht and eye colour.

na view ko na, yan nga yan, pareho tayo. ako inopen ko ang file sa MAC, trinay ko lang kung mattyps ko ang details, gumana naman, kaya ayun nakatype ang details sa appendix B ko.

Spousal sponsorship yung akin but sa form, di ko po nailagay information about my spouse, passport number, expiration date, etc.... Ayos lang kaya yon? Or May tendency kaya na mareject pa yung akin? Kahapon lang kase ako nagpasa ng passport ko. Nasa canada yung asawa ko.

Parang may nabasa ako na ganyang case. Try mo nlng mgcheck muna kasi hindi na ako sure. Pero kung ssundin ang instruction dapat inilagay mo. Anyway kung may kulang pwd mo nmn ihabol. Pwd ka rn tumawag sa CS nila just to make sure. Dati tawag ako ng tawag sa kanila.

Hello po s appendix b po b ilalagay name ng husband khit canadian citizen n siya at name ng baby applying for canadian citizenship kht hindi siya ksama s application? Thanks po
